Background

Medway Primary Care Trust has commissioned Social Care Strategies Ltd to:

  • Review the needs of all residents of the Community Support Service – implementing person-centred plans and enabling them to lead more fulfilling lives.
  • Ensure that the new service provides value for money and releases NHS revenue and capital for reinvestment in other services.
  • Support all staff affected by the changes throughout the process.
  • Ensure that future services are underpinned by fit for purpose commissioning arrangements.

The project is overseen by a Steering Group of senior executives representing the Primary Care Trust, Mental Health Partnership NHS Trust and Council. Social Care Strategies Ltd began its involvement by identifying with the Steering Group and other key stakeholders a vision of what a successful outcome would look like. A project plan has been established to make a sustainable reality of this vision.

This project is at an early stage of implementation. A detailed case study will follow in March 2008.
