Personalisation in action

Increasingly we are being asked to work with health and social care organisations to help them make a reality of their ambitions for personalised services. Our approach is simple – we begin by enabling people at all levels of the organisation to describe what success will look like, we find out what has stopped them doing it already, and then we make a plan to achieve their ambitions. Above all we make sure that people who use services and their carers have a real say in the development and implementation of this plan.

Person-centred day opportunities in North East Lincolnshire

North East Lincolnshire Care Trust Plus has ambitious plans for its services for people with learning disabilities. We have recently been asked to work with the organisation to help it make a reality of these plans. Our specific responsibility is to enable service users, carers, staff and managers achieve a shared vision for day opportunities – and then work with them to make a reality of their vision. Our approach is to embed person-centred planning tools within a structured approach to leading change. Contact us to find out more or to share your own experiences.

Redesigning acute mental health services in Medway

Medway Primary Care Trust needs to work out the number and type of acute mental health beds that it needs for the population of Medway and Swale. Social Care Strategies Ltd has been commissioned to design and lead this project. We will begin by running a workshop for service users and carers to learn about their good and bad experiences of using services. We will then engage all stakeholders using the workbook Laying the foundations for better mental health care. Contact us to find out more or share your own experiences.

Making a reality of user-led services in Staffordshire

Rockspur is a group of 20 or so people with learning disabilities in South Staffordshire. Their ambition is to become a user-led service. Social Care Strategies Ltd is supporting Rockspur to help them achieve their ambition. In practice this means helping them with a constitution, management structure and financial arrangements. Once Rockspur is up and running as an independent organisation, it will be Staffordshire County Council’s first genuinely user-led service. Contact us to find out more or share your own experiences.

Service user and carer conferences

Social Care Strategies Ltd has run conferences for between 100 and 200 people to give service users and carers a real say in the development and implementation of projects. These events bring real focus and momentum to projects – as well as improving outcomes by learning from people’s real experiences.
